OUR PHYLOSOPHY
Our aim is to provide you with support to enable you to live independently as possible in your own home. We aim to provide a service which is holistic, personal, responsive and respectful to your choices and rights and for us to work as a partnership with you.

OUR TEAM
THE ABOUT CARE SERVICES TEAM CONSITS OF:


Mavis Crabtree – director/proprietor (Responsible for running of the business)
Juliette Macdonald – Office Manager
Kay Winder – Administrator/ Trainer

Support Workers – Care Team (carry out day to day care duties)

MAKING A COMPLAINT/COMPLIMENT

If you are unhappy with the way your complaint is handled you can contact CQC (care quality commission for social care inspection) numbers are contained in your service user guide




WHAT IS CONTAINED IN THIS FILE?
This is information for you and in it contains information for you to make the most of your care package. The information recorded within this file about you is done so to ensure that we are able to provide you with high quality and safe care. Please read any information within the folder at any time.

1. CODE OF PRACTICE
This is expected conduct of our members of staff and our philosophy on how you should be treated regardless race, ethnicity, sex or religion.


2. STATEMENT OF PURPOSE
This document is a guide as to the tasks that ABOUT CARE SERVICES can undertake. It is also lists the senior staff, qualifications and how to contact us

3. YOUR INDIVIDUAL CARE PLAN

This contains your personal details, agreed care aims and the activities that you would like support with. This care plan is constructed with you.

4. RISK AND MANUAL HANDING PLANS

Attached you care plan will also be your manual handling plan (how to move you safely) and risk plan (how to manage any risks).

5. DAILY RECORD SHEETS
These are the records that are filled in on every visit. They provide a record of the tasks the carers have done on that visit and concerns or comments that need to be recorded. These are also collected monthly and returned to the office.

6. YOUR ASSESSMENT

When we come to meet you we carried out assessments to find out exactly what you require from ABOUT CARE SERVICES. These are kept in your folder and any reviews carried out. The assessments we carry out are as follows: general care Assessment, Manual Handling, Risk assessment and medication assessment.

7. QUALITY ASSURANCE

Please read this document as it explains what we do at ABOUT CARE SERVICES to ensure that you receive high quality care and the ways we try and improve.

8. SHOPPING RECORD

These are to be filled out each time a carer carries out shopping duties for you and you should be asked to sign this every occasion.


9. ACCIDENT / INCIDENT FORMS
Although we take steps to reduce all possible risks, these are in case of an accident that may happen when a carer is present on your property. This could be to you, to the staff member or to the property. These are filled out by the carer on the spot.
